ABOUT WEST NORFOLK COMMUNITY LEARNING DISABILITIES TEAM
West Norfolk Community Learning Disability Team provides health care services, health promotion advice, counselling and social care support for adults living with a learning disability. Most of the people live at home, but some people who receive assistance live in residential care settings. The service has its office close to centre of Kings Lynn.
The team is made up of staff from Norfolk County Council and Norfolk Community Health and Care Trust. Its members include a psychologist, specialist community nurses, occupational therapists and social workers. There are also ‘transition staff whose role is to assist young people when they leave education and need support to pursue their adult lives.
Although the team does provide some direct assistance to people, most of its activity involves arranging for, funding, supporting and monitoring services delivered by other health and social care providers.
