Transition Care is a newly established provider of Domiciliary Care especially created to give you a helping hand.
The aim of Transition Care is to provide support workers to service users in their own homes, who will assist and support individuals in promoting health, in its broadest sense, and maintain or improve the independence of our clients. Our goal is to provide a good value for money service that is reliable and responsive to the service user needs and preferences.
Transition Care understands the challenges of supporting someone with complex mental health needs. We also understand that with our specialist support and commitment to a recovery and independence ethos, clients can remain active and fully supported in their own homes irrespective of their level of need or dependency. We will give you that 'helping hand' towards independence and achieving your personal goals.
Transition Care aspires to create a friendly atmospere where the care and support we provide both enables and gives our clients the confidence to live as full, active and independent lifestyles as possible, we treat people as the individuals they are and thus we aim to ensure that every person who seeks our help has an individually designed, flexible quality care package meeting the unique needs and requirements that they have.
We ensure that at all times the services provided comply with the requirement of The Domiciliary Care Agencies Minimum Standards 2011.
