ABOUT LOCAL CARE HOME - THE TOGETHER TRUST COMMUNITY SERVICES
The Together Trust is a charity that provides a wide range of care, education and community support in the North West of England. Our community services are registered with the CQC as Together Trust domiciliary care services. We offer day services, short breaks, flexible outreach, community support and support at home. Our services are available to adults, young people and children with complex needs including autism, learning and/or physical disabilities and challenging behaviours. Many of the people we support have complex personal care needs.
Based in Stockport, our services are available to individuals and families across the North West of England and neighbouring areas.
Our outreach, community and home services offer:
Specialised support for children, young people and adults with complex needs - working in partnership with families/carers, often within their home environments
A comprehensive assessment process, including risk assessments and the development of support plans
Holiday, weekend and overnight breaks for individuals and families
Day services
Short break fostering schemes
A range of care services for disabled young people in crisis and medium and long-term care for those up to 30 years of age.
Community support – offering fun and enjoyment to help develop life skills and ongoing learning opportunities, promoting independence and empowerment. An emphasis is placed on being healthy, keeping fit, community involvement and enhancing peer/sibling relationships
Family and carer support services, including sitting services
A holistic service with continuity of approach for those accessing the Together Trust education and/or residential/supported living services
Individualised packages to support transitions; identifying future needs and requirements based on person centred planning
Personal assistant support to promote accessibility within education and the workplace
Supported tenancies
