ABOUT LOCAL CARE HOME - ST ANNE'S COMMUNITY SERVICES - ROCKHAVEN

Rockhaven is a large detached bungalow in a residential area of horsforth, Leeds. Within a short walk of the town centre. The home offers 7 single bedroom, lounge attached conservatory, dining room, kitchen 2 bathrooms and a utility room. 4 of the bedrooms are fitted with ceiling hoist, as is the lounge, this enables clients who need hoisting to be transferred easily and safely. There is a large secluded garden to the back of the property with easy access for those who need wheelchair/no step access.

The nursing staff are qualified in either learning disability or general nursing and are supported by a team of carers, who provide a high quality of care, using a holistic approach 24/7

A person centred approach is taken when implementing care and social care plans

The nursing team including care staff are trained in end of life care.

The home offers care to clients with differing health needs, which includes clients with dementia . staff are receive ongoing training to enable them to maintain the highest standard of care.

The service offers individual rooms for people who require nursing care and have a learning disability all the bedrooms are individually furnished and decorated.

The home consists of a lounge ,conservetory,dining room , kitchen 2 bathrooms and a utilty room. There is a secluded garden, clients are encouraged to access the local community with support from staff, some clients have support from outreach workers.all care offered is person centred and incudes day trips and short breaks. The home is staffed 24/7 and there is always a registered  nurse on duty with either one or two support staff, a cook and housekeeper. Access to a multi disaplinary team is easily accessable . the home has formed a good relationship with the local GP practice over many years.
