SLC Signposts is a provider of supported living and outreach services across Liverpool. We have a proven track record of innovation in the development and operation of ongoing support to people with a wide range of special needs – many of which are complex and diverse, including those with moderate to severe levels of learning disabilities, mental health needs, autism, physical disabilities, Acquired Brain Injuries, sensory needs and other associated complex conditions such as Cornelia De Lange Syndrome (CDLS).
The aim of our supported living and outreach support services are to provide support to adults and young people with a range of needs, enabling them to live in their own homes in the community. Our support is aimed at supporting individuals to:
Access and build on their community links
Gain new independent living skills
Identify and take up social/educational or training opportunities
Sustain their tenancies
Maintain their flats/accommodation
Increase their independence and self determination
Encourage greater and more positive participation in the community
Achieve personal goals
Move-on to independent living
