ABOUT LOCAL CARE HOME - SIGNHEALTH POLESTAR
Polestar provides provides a high quality supported living service for deaf people with mental health difficulties and/or complex needs. There are six self-contained flats in modern accommodation as well as a communal area. Individual packages of support and personal care are provided to tenants and they are encouraged and supported to achieve their individual potential. The project is staffed 24hrs a day by a team who are deaf and/or fluent British Sign Language users. Polestar is registered with CQC and as well as excellent CQC and Supporting People reports, receives excellent reports from care home inspectors.
Staff support residents to achieve new skills, improve self-esteem and become actively involved in the local and broader community through an agreed personalised care package. This package is reviewed on a regular basis.
Referrals can be accepted from agencies throughout the UK. They will be carefully assessed by the Manager who will work in partnership with other healthcare and social care professionals. Once a referral has been accepted and funding secured, staff will encourage and support the new resident to be actively involved in the transition to their new home.
