Short Term Support and Rehabiliatation Services
"A short-term service to promote reablement and maximise independent living at home."
We provide a 24 hour response, 365 days a year, to residents of the Royal Borough of Windsor and Maidenhead or registered patients at a GP in Windsor, Ascot and Maidenhead CCG.
Our services are for adults aged 18 and up, who need:
Short term rehabilitation service to maximise independence. - This may be needed following discharge from hospital when an adult has had an operation, procedure or illness that limits their abilities in the short term.
Crisis response social care - This may be needed if a main carer (eg. family member) becomes unwell or if an adult’s wellbeing is at risk and without help they would need admitting to hospital or long-term care.¨
Palliative care support – We provide support to adults with life-threatening illnesses, including care to adults who are thought to have less than 6 weeks to live. We support adults who wish to be at home or spend some time at home prior to admission to a hospice.
