ABOUT LOCAL CARE HOME - REABLEMENT


Reablement means giving people over the age of 18-years-old the opportunity, motivation and confidence to relearn/regain some of the skills they may have lost as a consequence of poor health, disability, impairment or accident.

The service provides intensive reablement for a maximum of six weeks in advance of home care services being provided. After six weeks, long term home care will be provided by an alternative independent sector provider if you are still eligible.

The service also offers a chance to gain new skills that can help develop and maintain independence.

As well as giving you the satisfaction of being able to do more things for yourself, input from the Reablement Service is likely to lead to less need for traditional home care in the long-term.

Who is eligible for the service?Access to the service will be through a social worker/care co-ordinator assessment which identifies that:

You have been assessed as being eligible for a home care service.
You are an older person or an individual over 18-years-old who has a physical or learning disability.
You may benefit from assistance with the development of skills for more independent living.
You accept that reablement is a specialist home care service which is chargeable.
You accept that the Reablement Service is a stepping stone of intensive reablement which is provided up to a maximum of six weeks (but can be considerably less) and that long-term home care needs will be provided by an alternative independent sector provider after the period of reablement if ongoing needs are identified.
