ABOUT LOCAL CARE HOME - REABLEMENT
The Reablement team provide a person centred support service to people living in their own homes in Peterborough. The focus of the work is a short term, intensive support to help people recover their independence, usually following periods of illness and/or hospitalisation. Support is generally provided for a period of up to six weeks.
