ABOUT LOCAL CARE HOME - PHOENIX HOUSE
Phoenix House is a well established service which provides care and support to nine adults with learning disabilities and physical disabilities. Phoenix House is located in Holt, ten minutes from Trowbridge town centre.
The poeple who live here lead an active life accessing local community based activities and resource centres. Their person centred plans are individually tailored and describe clear outcomes. Care plans are tailored to assist with individual needs around personal care and mobility difficulties and encourage choice and independence.
The environment has been carefully designed to respond to the needs of adults with a learning disability and physical disabilities, whilst providing an environment that is warm and homely. There are nine bedrooms, all with en-suite facilities. Each room is fully furnished to a high standard and we encourage people to bring their own belongings and to decorate their rooms in the style they prefer.
The lounge and day room are ideal environments in which to relax or take part in many of the activities provided by the service. The open plan kitchen includes a low level sink and work space to encourage everyone to participate in the preparation of meals, supported by a member of staff. The service is surrounded by gardens with a number of features.
