ABOUT LOCAL CARE HOME - INTERMEDIATE HOME CARE - STOCKTON
Intermediate Home Care provides support to people living in their homes for up to six weeks. This support is usually provided following people having surgery and need support to regain their independence. It also provides support where district nurses or GP's have identified potential crisis situation, such as a person having a fall at home and sustaining an injury. The service works in conjunction with health, who provide a rapid response team and physiotherapy team.
