ABOUT LOCAL CARE HOME - HOMECARE REABLEMENT SERVICE
HomeCare Reablement Service is provided by the London Borough of Hammersmith and Fulham. It is a service that provides a short term package of care designed to support people regain their confidence and independence at home following illness or hospital admission. The service provides multidisciplinary skills such as rapid response nurses, occupational therapists and skilled reablement officers all of whom contribute, as necessary to the assessment of people's needs.
