ABOUT LOCAL CARE HOME - CROSSROADS CARE IN MID YORKSHIRE
Crossroads Care in Mid Yorkshire provides care to carers. Our vision is that every carer is recognised, supported and offered services to help them maintain their own health and well-being.
We mainly do this by providing short break services that meet the needs of Carers and the people they support. To support Carers, we provide care to children and adults in their own homes in Kirklees, Wakefield, Calderdale and South Leeds.
We are committed to providing high quality person-centred care and support to meet the needs of carers and the people they care for. We work in partnership with the carer and the person with care needs to design a support plan that meets their needs and aspirations. Promoting independence, choice and ensuring dignity are at the heart of all we do.
Services are provided for children, young people and adults across a wide range of needs and conditions including multiple sclerosis, Parkinson's disease, stroke, physical disabilites, dementia, learning disabilities, autistic spectrum disorder and sensory impairments.
Services include:
Assistance with personal care - eg bathing, washing and toilet needs
Prompting, assisting with or administering medication
Planning and preparation of meals
Supporting service users outside their home - eg to attend appointments, enjoy leisure activities and to go shopping
Complex healthcare and specialised tasks - eg PEG feeding
End of life care
Light domestic duties
We provide a Day Centre in Kirklees on Saturdays for children with a range of disabilities.
Our dedicated professional staff are Level 2 Diploma/NVQ qualified in Health and Social Care and undertake a range of more specialised training.
We are a Network Partner of Carers Trust, Britain's leading carer support charity, formerly The Crossroads Care Association and The Princess Royal Trust for Carers.
