ABOUT LOCAL CARE HOME - CREATIVE SUPPORT - CHORLEY SERVICE
The service supports people who have a learning disability, in their own home, providing personal care in line with a supported living model. The service also runs an outreach programme and provides respite care services. People who use the service have their own tenancies and receive their support from people employed by Creative Support, provided in line with people's individual needs throughout the day and night.
