ABOUT LOCAL CARE HOME - CRAEGMOOR THE BIRCHES
The Birches is a residential placement for adults requiring specialist support for their mental health needs, including personality disorders. This may also include individuals who have a learning or physical disability.
The service will accommodate seven adults from the age of 18 and upwards. Many of these adults may require additional support in terms of their relational and environmental security in moving back into the community.
The service is highly staffed with each staff member undergoing basic training in mental health awareness.
Each individual will have access to all community facilities, including general health and specialist mental health services; which may include regular contact with a community multidisciplinary team. The service will also look to incorporate a specialist lead to ensure all individuals are offered support in therapeutic engagement in a variety of community activities. The aim is to facilitate skills development and progression through the care pathway to include social inclusion.
We help to progress people through this rehabilitation process and move onto their own supported accommodation, where we can assist in the continuity of support if required via our Supporting You in East Midlands branch. The process is different for each individual with progression lasting around 24 months. For some individuals it may be appropriate to look at a longer term placement. The service will only move people with the appropriate support in place.
