The Central Reablement Service & Home Pathway service provides reablement services to adults with physical disabilities and older people with high dependency needs. The service aims to meet peoples individual needs in their own home, offering choice to individuals in a manner that promotes and encourages their independence. The service provides short term intensive support to people recovering from an illness or a period of time in hospital. The service operates across Manchester.
