Carlton Avenue is a specialist residential service for adults with profound and multiple learning disabilities, physical disabilities and complex health needs. This fully adapted service supports service users to develop person centred that promote increased choice, control over their lives and independence.
Service users are supported to reach their full potential by experiencing new interests, developing new skills and by participating in community life.
Our experienced team works in partnership with families, carers, advocates and public service professionals to ensure that the support provided is pro-active and appropriate.
We actively follow the principles of person centred planning and person centred active support. Service users are supported to make informed choices about their lives, take part in meaningful activities and achieve their planned outcomes.
We support service users to develop individually tailored Health Action Plans which focus on increasing independence and developing communication, social and life skills.
Our Total Communication approach enables staff to implement individualised communication strategies for service users with severe communication and sensory difficulties.
Activities focus on developing social and communication skills to enable each service user to reach their full potential. Service users are supported to maximise their independence by accessing social, educational and leisure activities and by developing daily living skills within the home.
