ABOUT LOCAL CARE HOME - CARE MANAGEMENT GROUP - 97 OLD STREET
The provider is registered to provide the following regulated activity: Accommodation for persons who require nursing or personal care. The service provides accommodation and personal care for up to five people who have learning disabilities or autistic spectrum disorder. This service is not registered to provide nursing care. The service has a registered manager.
Old Street is a specialist residential service for adults with severe learning disabilities, complex health needs and limited mobility. Service users are supported to develop individualised person centred plans and health action plans that promote increased choice, control over their lives and independence.
Our experienced team works in partnership with families, carers, advocates and public service professionals to ensure that the support provided is pro-active and appropriate.
We follow an active person centred approach which supports service users to make informed choices, take control of their lives and achieve their planned outcomes.
Our Total Communication culture enables staff to implement individualised communication strategies for service users with severe communication and sensory difficulties.
Service users are supported to develop individually tailored Health Action Plans which focus on supporting people to manage their healthcare needs and increase their independence.
We actively encourage personalisation and welcome enquiries and self referrals from service users and their supporters in addition to referrals from local authority teams.
