16 Hawthorn Crescent is run by leading provider CMG Ltd. It is a specialist residential service for adults with moderate to severe learning disabilities, physical disabilities and complex health needs. Service users are supported to develop individualised and accessible person centred plans and health action plans that promote increased choice, control over their lives and independence.
CMG's experienced team works in partnership with families, carers, advocates and public service professionals to ensure that the support provided is pro-active and appropriate.
16 Hawthorn Crescent is a fully adapted bungalow with four bedrooms. There is also a communal living room, dining room, kitchen and a large garden.
We follow an active, person centred approach which supports service users to express their wishes, make choices and achieve their planned outcomes.
We support service users to develop individually tailored Health Action Plans which focus on increasing independence and developing communication, social and life skills.
Our Total Communication approach enables staff to implement individualised communication strategies for service users with severe communication and sensory difficulties.
We actively encourage personalisation and welcome enquiries and self referrals from service users and their supporters in addition to referrals from local authority teams.
