ABOUT LOCAL CARE HOME - BARNSLEY COUNCIL HOME ASSESSMENT AND RE-ABLEMENT TEAM
Barnsley Council 'Home Assessment and Re-ablement Team' (HART) is registered to provide personal care for people who require support in their own homes following a period of illness so that they may continue to live independently. The service provides support for people until they gain independence or until the sixth week where an assessment will be made to establish whether the person still requires further support from a more permanent service.
